On-site educational sessions and webinars about advance care planning and LaPOST are regularly scheduled for health care professionals such as physicians, nurses, social workers, chaplains and administrators. In order to enhance the LaPOST model of care, the Quality Forum has partnered with Vynca to allow health care providers across multiple care delivery continuums in Louisiana to electronically complete, store and access patients’ end-of life wishes. This collaboration will help enhance the state’s current advance care planning program, which includes the LaPOST form.

Talking about goals of care and end-of-life decisions with residents and caregivers in long-term care facilities can be difficult and uncomfortable at times. Still, these conversations are vital for residents, caregivers and the health care team. To support health care professionals, Susan Nelson, MD, will present a series of on-site seminars focusing on different aspects of advance care planning and the LaPOST document.
