The Louisiana Physician Orders for Scope of Treatment (LaPOST) document is a quality initiative of the Louisiana Health Care Quality Forum, a private, not-for-profit organization dedicated to reshaping health care in Louisiana. Approved as Act 954 in the 2010 regular session of the State Legislature, LaPOST is an evidence-based model designed to improve end-of-life care for those with serious, advanced illnesses.

In 2018, we partnered with Vynca to develop a statewide eRegistry, enabling clinicians to electronically complete and submit forms, query registry of any form, access forms in real-time, and receive notifications with EHR integration.

Dr. Susan Nelson
MD, FACP, FAAHPM
Nelson is the LaPOST Coalition Chair and board-certified in internal medicine, geriatrics and hospice and palliative medicine. She serves as Medical Director of Post-Acute Care for the Ochsner Health Network and is a Palliative Medicine Physician with Ochsner Health System.
